P-291 Missing colorectal liver metastases after neoadjuvant chemotherapy: Can we improve the prognosis?

Complete disappearance of colorectal liver metastases (CLM) after chemotherapy is considered a good prognostic factor survival, but the presence residual tumoral cells arrives to 80% and recurrence rate 75% patients. Nowadays, management these patients still controversial. The aim this study evaluate impact different onco-surgical strategies in missing (MM) for who undergo curative surgery. This retrospective single-center (1999-2019) includes 69 with ≥1 MM. have been divided into 2 groups: Patients hepatic arterial infusion (HAI) (HAI group) without HAI (non-HAI) Overall survival (OS), progression free (PFS), (LiR) local (LoR) analyzed each group. In total, 312 MM 779 initial CLM identified median number (1-5) MM/patient. Ten (14%) were resected original site complete pathological response (TRG1) 20% (n=2). 5y-LiR was 55% vs 79% (NS) both groups non-HAI groups). 5y-PFS 5y-OS 51% 38% et 59% 65% (NS). Highly chemo sensitive present favorable long-term outcomes survival. high makes consider that resection sites strongly recommended when it possible. surgery does not significantly decrease risk presenting great chemotherapy.
